Co-Founder & Chief Executive Officer

Lusilda (Lucy) Agolli, MHA, CHW, is the Co- Founder and CEO of CareLink Services and Billing Solutions, LLC, where she focuses on advancing Community Health Worker (CHW) integration, workforce development, and sustainable care models. With extensive experience in population health, care coordination, and practice transformation, she partners with healthcare organizations to design and implement programs that improve health outcomes and strengthen community-based care. Ms. Agolli also serves as a CHW Success Coordinator and Instructional Educator at Practice Transformation Institute. In this role, she supports initiatives including grant writing, program coordination, relationship development, curriculum design, and program evaluation. She facilitates key components of the CHW training curriculum, including social determinants/drivers of health (SDOH) screening and assessment, Z-code documentation and billing, care coordination workflows, and effective documentation practices. She also provides mentorship and structured feedback to learners to strengthen practical skills and support the successful application of CHW competencies in healthcare and community settings. Prior to joining Practice Transformation Institute, Ms. Agolli spent more than eight years at Medical Network One (MNO), where she served as a Community Health Worker specializing in care coordination, quality improvement, and patient outreach and engagement. She later became the CHW Program Lead and Instructor, contributing to the design, development, and implementation of the CHW program. Ms. Agolli is actively involved in advancing the CHW profession through policy and advocacy. She contributed to the statewide CHW Advisory Council coordinated by the Michigan Department of Health and Human Services and participates in the National CHW Policy Working Group coordinated by the National Association for Community Health

Workers and Partners in Health United States. She holds a Master of Health Administration from Eastern Michigan University and a Bachelor’s Degree in Health Science with a concentration in Population Health Care Management from Madonna University. She earned her Community Health Worker Certificate and Community Health Worker Instructor (CHWI) Certificate through the Texas A&M School of Public Health Center for Community Health Development’s National CHW Training Program.

Co-Founder & Chief Operating Officer

Anjeza Muhameti, MHA, CPC, CMRS, RHIT, is the Co-Founder and Chief Operating Officer (COO) of CareLink Services & Billing Solutions, LLC, where she leads operational strategy, revenue cycle management, and organizational infrastructure development. She specializes in medical billing, credentialing, compliance workflows, and building scalable systems that support Community Health Worker (CHW) programs, primary care providers, and community-based organizations.

With extensive experience in healthcare operations and administration, Ms. Muhameti partners with organizations to design and implement efficient billing systems, streamline payer enrollment processes, and optimize financial performance. Her expertise includes Medicaid and HMO credentialing, CHAMPS enrollment, workflow development, and operational oversight to ensure sustainable and compliant healthcare delivery models.

Ms. Muhameti currently serves as a Medical Office Manager and Billing & Coding Manager, where she oversees clinical and administrative operations, revenue cycle performance, staff management, and compliance initiatives. She has played a key leadership role in practice transformation efforts, including advancing Patient-Centered Medical Home (PCMH) standards and improving care coordination processes.

In addition, she brings both outpatient and inpatient healthcare experience through her work with Corewell Health, further strengthening her understanding of healthcare systems, patient flow, and interdisciplinary coordination.

Ms. Muhameti holds a Master’s in Health Services Administration and a Bachelor’s Degree in Health Science with a concentration in Health Services Administration. She is a Certified Professional Coder (CPC), Certified Medical Reimbursement Specialist (CMRS), and Registered Health Information Technician (RHIT). She is committed to advancing operational excellence, strengthening healthcare infrastructure, and supporting innovative, community-focused care delivery models.

 Dr. Saima Khan is a board-certified specialist in Internal Medicine with over 20 years of clinical experience providing comprehensive care to adult patients. She earned both her undergraduate degree and Master of Public Health from the University of Michigan in Ann Arbor before completing her medical degree at Wayne State University School of Medicine.

Dr. Khan completed her residency in Internal Medicine at Advocate Illinois Masonic Medical Center in Chicago, where she received several peer and faculty awards in recognition of her clinical performance and dedication to patient care.

After completing her residency training, Dr. Khan returned to Michigan and joined Family Practice Care, PLLC in Warren, where she has continued to provide comprehensive primary care services. As an internist, she focuses on the prevention and management of chronic conditions such as diabetes and hypertension while also providing preventive and wellness care for her patients.

Dr. Khan is committed to building strong patient relationships and emphasizes patient education as an important part of improving long-term health outcomes. She believes in a patient-centered approach to care that supports both the physical and emotional well-being of those she serves.

Dr. Khan is board certified by the American Board of Internal Medicine and holds hospital privileges at Ascension Macomb-Oakland Hospital and Beaumont Hospital Grosse Pointe. She speaks both English and Urdu.
